Since 1994, it has been the mission of Connally Memorial Home Health to provide quality, progressive care to patients in the place they need it the most – the comfort of their own home. This care includes helping patients recuperate from surgery, injury, or illness, learn about their disease process, receive therapy after a stroke, or have their medication regimen monitored.

QUALITY CARE

Our agency provides all levels of service, from skilled nursing care to home making, and there is always a registered nurse on call. This type of care is provided in the comfort of the patient’s home. Connally Memorial Home Health is fully licensed and certified under federal, state, and local laws, and is approved for participation in Medicare and Medicaid programs.

 


CRITERIA FOR ELIGIBILITY

Home health care is available to any patient, providing this individual:

    • Has a physician’s order prescribing home        health services
    • Has a need for skilled health care because of        illness or injury
    • Is essentially homebound

SERVICES

Connally Memorial Home Health offers a variety of health care services to meet the individual needs of each patient. All services are provided by licensed or certified professionals who have a special interest in caring for people in their home environment.

 

SKILLED NURSING CARE

A registered or licensed vocational nurse provides treatments and procedures requiring specialized nursing skills. This may include: wound care, dressing changes, blood pressure monitoring, diabetic care, medication management, and infusion therapy. Our nurses play an important role in educating the patient on their condition and teaching family members to provide care for the patient.

SPECIALIZED THERAPY

A licensed physical, occupational, or speech therapist can offer specialized care to help the patient improve or restore functions impaired or lost because of illness or injury. The therapists can tailor a program to meet the patient's specific needs.

SOCIAL SERVICES

A social worker provides support to the patient and family members in dealing with the lifestyle changes caused by illness or injury. Social workers can also refer patients to appropriate community resources.

HOME HEALTH AIDE CARE

A certified nursing aide assists the patient with personal care under the direct supervision of a registered nurse or therapist. Aides are available to help with tasks such as bathing and personal hygiene.

PRIMARY HOME CARE

A MEDICAID PROGRAM ADMINISTERED BY THE TEXAS DEPARTMENT OF AGING AND DISABILITY SERVICES (DADS). A PRACTICTIONER’S STATEMENT OF MEDICAL NEED FOR PHC IS REQUIRED.

A DADS caseworker and a provider agency non-medical supervisor screen you, the client. Then we begin reviewing your needs through discussions with the DADS caseworker and our staff. Next, a DADS caseworker develops an individualized program to manage your daily needs. We match your needs to the best possible caregiver and bring them to your home. After the caregiver has been placed in your home, we continue our unique system of continued personalized contacts through telephone check-in and home visits. Most important, we continually communicate with the caregiver who is providing care to you.

• Hygiene assistance • Toileting
• Light housekeeping • Bathing
• Errands and shopping • Many other tasks

DIABETES SELF-MANAGEMENT EDUCATION PROGRAM

WHAT IS DSME?

The Diabetes Self-management Education program offers comprehensive diabetes services to patients with diabetes and their loved ones. Patients will be taught self-management techniques and will be provided assistance in understanding their health condition.

Our team of registered dieticians and registered nurses will work the patient’s primary physician to help:

• plan meals
• monitor blood glucose levels
• self administer medication
• handle emergencies
• prevent complications
